Form Building

Once your department has identified the need to create a Dynamic Form, we ask that a Key Stakeholder be assigned as a single point of contact and dedicated resource to work with our Dynamic Forms ITS Team throughout the creation process of your form.

Dynamic Forms allows each department to build their own forms after being trained by the Dynamic Forms ITS Team, providing creative control with each form, and receiving guidance along the way. 

• Timeline: The Timeline of building a form is dependent on the availability of the Dynamic Forms ITS Team, and you and your teams time that you're able to dedicate to building forms within Dynamic Forms. 
• Level of Engagement and Support: The Dynamic Forms ITS Team will guide you through your first form from start to finish. Ultimately, you are responsible for designing, building, testing, administering and maintaining your form after the completion and launch of your first form. Please note that the Dynamic Forms ITS Team is available for additional support as requested to ensure your success in utilizing Dynamic Forms.
